All mentors are screened through an intensive assessment process including: interview, training, background check, and references.

Eligibility: "- Veteran, one day active duty
  - Must not have received a dishonorable discharge
  - Homeless or at imminent risk of homelessness.
  - Meet low-income program guidelines for HH size (50% AMI varies by county)
  - Not already receiving services from another SSVF program. DD214, VA ID or a letter from the VA.
  -  SSVF is able to use a system to look up veterans so them not having proof of veteran status is not a barrier to our program.  If we are unable to find them in that system, which is rare, then they do need to provide alternate documentation.

Eligibility: All students, regardless of insurance/ability to pay, may access the PCHS NMSD SBHC through self-referral.  While students are encouraged to establish overall primary care services with PCHS as part of integrated/whole-person care model, PCHS primary care services are not required for SBHC services.  Confidentiality consistent with state and federal law is offered to all students.

These services help veterans get and keep permanent housing. The Veteran must use case management services to receive a HUD-VASH housing voucher; If a Veteran is found eligible, a HUD-VASH case manager will help the Veteran apply for a housing voucher and will refer the Veteran to the right PHA.
